HOW TO FILE A TITLE VI COMPLAINT
Any student, employee, applicant, or visitor may file a Title VI report. To begin the process, you may contact the Title VI Coordinator directly or submit a written complaint online. If students contact faculty or staff, they can be referred to the Title VI Office. See the Title VI reporting form below:
PROCESS FOR TITLE VI OR BIAS-RELATED COMPLAINT
- The form goes directly to the Title VI Coordinator who investigates the issue, concern, or complaint. If warranted, the Title VI Coordinator can collaborate with the BIT/Bias Response Team, specifically if it involves students. The Coordinator does an initial review of the report. This typically takes 2–3 business days. Information is shared with offices (i.e., Human Resources, Public Safety, etc.) as needed. The goal is to maintain confidentiality while resolving the reported concern.
- The report is assigned to the appropriate campus official for investigation and/or follow-up action. Witnesses may be contacted to gather information.
- An email/letter is generated to those reporting the incident, including written notice of available campus resources.
- Appropriate campus officials investigate the reported incident. They may contact the complainant and/or appropriate witnesses. Email/letter notification is forwarded to the parties.
- All contents of the report are reviewed using existing SUNY, federal, state, and local policies, procedures, and guidelines, including the ËÄ»¢Ó°ÊÓ Student Handbook, Title VI procedures, discrimination policy, and Human Resources guidelines.
- The officials report back to the Title VI Coordinator on the status of the report and any recommendations.
- Student incidents may be reviewed for action through the Student Code of Conduct.
- The database is updated. Statistics are maintained and offered for annual reporting in accordance with SUNY and state guidelines.
- All involved parties are notified of findings and outcomes.
